would someone have a strap button for the under side of the guitar from a concord or other models that type of button for sale?

I don't think they are still available, the closest I've found were the same shape but too small a diameter.

From what you've described it sounds like the screw is bent, possibly because it was allowed to work loose and the stress from the pull on the strap just warped it. Remove it. Refill the hole and reinstall the button with a new screw.
If the button itself is bent that's a replacement item, but I don't understand what would have caused it to deform, especially on the upper bout of the guitar. Very unusual.

They're all made of brass and plated. Those on ebay are chrome , ask the seller!
To bend one of those will take some serious hammering. More than likely a bent screw as mentioned.
If you don't know what your doing take it to a guitar shop.

I ordered the set/pair that Westone sent a link to on ebay. If anyone is looking for just 1 of these let me know? maybe I can help you out too? but crossing fingers that one will be enough >.< ?
Yeah i'm not sure too how that part along with the screw got bent Barry o.O ? but maybe was when the guitar was rested on the actual button itself? well for the under part of the guitar that is, not sure about the upper bout of the guitar though? yikes ? o.o

If it's a gigging guitar, replace the endpins with strap locks!!
